What causes the "unable to read XXXXXXXX bytes" error?

Answer
There is only one possible cause for this error: the optimized PHP file is uploaded in ASCII mode rather than BINARY mode. Because most FTP clients treat .php files as ASCII (which they normally are) you will need to override the transfer mode.
